The International Prostate Symptom Score is the seven-question scale urologists use worldwide to measure how much urinary symptoms are actually costing you. Answer for the past month on a 0 to 5 scale, where 0 means not at all and 5 means almost always; for the nighttime question, 0 means no trips and 5 means five or more.

Common questions about the IPSS
What is a normal IPSS score
0 to 7 counts as mild, 8 to 19 moderate, and 20 to 35 severe. Most men with no prostate trouble score in the low single digits. The score counts how often symptoms happen, not how much they bother you, which the IPSS asks separately: a 10 that ruins your sleep can matter more than a 12 you barely notice.
At what IPSS score should I see a urologist
Eight or higher is the conventional threshold for an evaluation, and any score is worth a visit if symptoms disrupt sleep, work, or travel. Sudden inability to urinate, blood in the urine, or fever should be seen promptly regardless of score.
Is the IPSS a diagnosis
No. It quantifies symptoms, not causes. An enlarged prostate is the most common driver in men over 50, but the same score can come from bladder or other issues, which is why the evaluation includes a prostate measurement and a check of how well the bladder empties.
How is the score used in treatment decisions
